The gilt cupro-nickel medal showcases the right-facing effigy of Queen Elizabeth II, wearing St Edward's Crown. The reverse side displays the shield of the UK Royal Coat of Arms. The shield is flanked with the dates 1952 and 2002, outlining the 50 years since ascension.

The ribbon, 32 millimeters wide, is vibrant blue with red edges, and a central line of white bisected with a thin stripe of red.
