The obverse features the right-facing effigy of Queen Elizabeth II wearing the St Edward's Crown and the inscription 'ELIZABETH II DEI GRATIA REGINA FID. DEF.'. The reverse bears the inscription "THE 25TH YEAR OF THE REIGN OF QUEEN ELIZABETH II 6 FEBRUARY 1977". The inscription is circled with a crowned wreath.
The accompanying ribbon, 32 millimeters wide, features a central line of blue bisected with a thin red stripe. The blue is bordered with white, and red on the edges.
